Contact your HR to fill a Prudential Enrollment form and attach a copy of your national ID or driving permit or passport or birth certificate for each eligible family member to Prudential. For Children under 3months, attach a birth certificate or Notification form or discharge form, 4 months to 18years a birth certificate or passport
