- A client visits the Prudential Uganda office or emails requesting to make a Surrender Claim.
- At this stage only the client’s policy number is provided for verification.
- Documents required to start the process are; ID proof of claimant, bank details of claimant and original policy documents.
- If the client paid premiums through Direct Debit, a cancellation form is shared along with the Surrender Claim form.

Surrender Claims Document (2 downloads )
- Beneficiary/Next of Kin notifies Prudential who shall immediately acknowledge receipt of the same.
- Claimant submits all necessary requirements. (Claim intimation form, Certified Copy of Death Certificate from NIRA, Original policy document, ID of Claimant and that of the Life Assured if it was never submitted at inception, Police Report (for only accidental death), Medical documents prior to death)
- After review of claim, Prudential shall issue a Discharge Voucher.
- Claimant shall sign and return Discharge Voucher.
- Payment is made within 5 working days on receipt of signed Discharge Voucher.

Death Claim (8 downloads )- Claimant/Client notifies Prudential who shall immediately acknowledge receipt of the same and advice on the necessary requirements.
- Claimant submits all necessary requirements. (Full Medical Examination Report, Original policy document, ID of Life Assured and of the authorized Claimant, Police Report)
- On receipt of all requirements Prudential shall issue a Discharge Voucher.
- Full Medical Examination Report.
- Original policy document.
- Police Report.
- Completed Intimation Form (this is provided by Prudential).

Permanent Total Disability (9 downloads )- .Our Claims Officer contacts the Policy holder two months prior to the maturity date to notify the client on the necessary requirements needed.
- Claimant submits all necessary requirements. (policy document, national ID and direct debit cancellation form)
- On receipt of all requirements Prudential shall issue a Discharge Voucher.
- Claimant shall sign and return Discharge Voucher.
- Payment is made within 5 working days on receipt of signed Discharge Voucher.
